JOB SUMMARY

The Triage Support Worker provides quality frontline assistance to vulnerable guests. The role will include supporting people to access a range of Exodus programs and services.

Specifically, this role involves:

  • Provide clear, accurate and relevant resources, and program information (including on site programs, services and eligibility requirements) to individuals, staff and external stakeholders.
  • Referring guests through to casework, counselling, clinical and pastoral support; ensuring mental, physical, emotional, and spiritual wellbeing of guest is appropriately catered.
  • Supporting the delivery, consistency, and quality of general practice care and allied health supports.
  • Greet incoming guests and clients to the Exodus Foundation clinic and community space, triaging enquiries or directing guests to the appropriate person they are meeting with.
  • Participate in community engagement activities as directed and promote the mental health and wellbeing of the community
  • Liaising with, and referring to, internal and external service providers for guests while providing emotional support and relief.
  • Liaising with caseworkers on intake assessments and assisting with interview scheduling.
  • Assisting With EAPA and Food parcel assessments and collection of paperwork required.
  • Supporting on mobile outreach for Health and Wellbeing and Food Services.
  • Assisting with compliance of medical accreditation and ensuring adequate cold chain procedures for vaccinations.
  • Provide information about Social Health and Wellbeing support and broader Exodus services.
  • Update client details and enquiries as required within the CDS database
  • Forward telephone calls and emails to Exodus Staff as required
  • Provide personal care items to guests
  • Provide take away meals to guests

 

The successful applicant will have:

  • Cert IV in Community Services or equivalent
  • Previous experience as receptionist in community service or medical/health care environment
  • Moderate computer and database skills
  • Basic knowledge of WHS standards and practices
  • Ability to multitask under pressure
  • Excellent communication skills
